People with obstructive sleep apnea (OSA) have disrupted sleep and low blood oxygen levels. When obstructive sleep apnea occurs, the tongue is sucked against the back of the throat. This blocks the upper airway and airflow stops. When the oxygen level in the brain becomes low enough, the sleeper partially awakens, the obstruction in the throat clears, and the flow of air starts again, usually with a loud gasp.
Repeated cycles of decreased oxygenation lead to very serious cardiovascular problems. Additionally, these individuals suffer from excessive daytime sleepiness, depression, and loss of concentration.
Some patients have obstructions that are less severe called Upper Airway Resistance Syndrome (UARS). In either case, the individuals suffer many of the same symptoms.
The first step in treatment resides in recognition of the symptoms and seeking appropriate consultation. Oral and maxillofacial surgeons offer consultation and treatment options.
In addition to a detailed history, the doctors will assess the anatomic relationships in the maxillofacial region. With cephalometic (skull x-ray) analysis, the doctors can ascertain the level of obstruction. Sometimes a naso-pharyngeal exam is done with a flexible fiber-optic camera. To confirm the amount of cardiovascular compromise and decreased oxygenation levels, a sleep study may be recommended to monitor an individual overnight.
There are several treatment options available. An initial treatment may consist of using a nasal CPAP machine that delivers pressurized oxygen through a nasal mask to limit obstruction at night. One of the surgical options is an uvulo-palato-pharyngo-plasty (UPPP), which is performed in the back of the soft palate and throat. A similar procedure is sometimes done with the assistance of a laser and is called a laser assisted uvulo-palato-plasty (LAUPP). In other cases, a radio-frequency probe is utilized to tighten the soft palate. These procedures usually performed under light IV sedation in the office.
In more complex cases, the bones of the upper and lower jaw may be repositioned to increase the size of the airway (orthognathic surgery). This procedure is done in the hospital under general anesthesia and requires a one to two day overnight stay in the hospital.

Oral appliances are an excellent treatment for patients who snore or who have mild to moderate sleep apnea. They are a comfortable and effective treatment for those patients who have difficulty with a the claustrophobic feeling or other side effects of a CPAP machine. He fabricates several different types of oral appliances dependent upon the patient’s symptoms and occlusion of their teeth. The Micro2 oral appliance is among those types that he utilizes.

I have had dental implants for thirty years due to a car accident. I was having severe pain and irritation from my dental implant appliance. Dr Weirerbach knew exactly what was needed, it didn't fit properly and the hardware was outdated. The temporary appliance fit better than the actual appliance I had from a prior dentist. I have my permanent appliance now and it is absolutely beautiful and better yet it fits perfectly. My teeth look real and very much like my actual teeth looked. I am very anxious at the dentist and Dr Weirerbach and all of his staff were very accommodating and kind. My teeth were so terrible I was petrified even to think of going anywhere to have a consultation I found myself not smiling, I won'tdidnt want to socialize so one day I ran into a friend of mine I hadn't seen for a long time she had the most beautiful smile so I asked her who is your dentist her reply was doctor John Weierbach 2nd she said hes not just a Dennist he does it all.So I looked him up on his website and wrote an email I kept that he mail for 2 weeks as a draft it was so hard for me to hit that send button because I was so ashamed of my teeth! I went to Dr Weierbach because I was unhappy about how my current orthodontist was treating my bruxism (teeth clenching at night). He assessed me and suggested trying Botox to relax the muscles. Within MINUTES, I felt relief and my face looked thinner as the muscles relaxed (turns out I was totally ripped, but only on my face, hahaha!). I slept with no guard and no medicine for bruxism for the first time that night in over 20 years. Night two has also passed. I am sleeping better and I have no pain in my jaw in the morning. Literally life-changing!

We have been seeing Dr. Weierbach since the 1980's. Our entire family has been patients there. A friend of ours had her front tooth broken off in a floor hockey game. She went to Weierbach for emergency treatment. The crown he made for her was perfect! She still tells me about the dentists she has seen since then that didn't recognize her front tooth had a crown! I met Dr. Weierbach when I was 18 years old, and a pre-dental college student. Today, I am 23 and will be starting dental school in a couple of weeks! Without a doubt, Dr. Weierbach has been the most positive influence in my journey and truly a mentor figure. Of all the dental professionals I have shadowed in Lancaster, I could quickly tell he not only LOVES what he does, but he also EXCELS at it.
