People with obstructive sleep apnea (OSA) have disrupted sleep and low blood oxygen levels. When obstructive sleep apnea occurs, the tongue is sucked against the back of the throat. This blocks the upper airway and airflow stops. When the oxygen level in the brain becomes low enough, the sleeper partially awakens, the obstruction in the throat clears, and the flow of air starts again, usually with a loud gasp.

Repeated cycles of decreased oxygenation lead to very serious cardiovascular problems. Additionally, these individuals suffer from excessive daytime sleepiness, depression, and loss of concentration.


Some patients have obstructions that are less severe called Upper Airway Resistance Syndrome (UARS). In either case, the individuals suffer many of the same symptoms.

The first step in treatment resides in recognition of the symptoms and seeking appropriate consultation. Oral and maxillofacial surgeons offer consultation and treatment options.

In addition to a detailed history, the doctors will assess the anatomic relationships in the maxillofacial region. With cephalometic (skull x-ray) analysis, the doctors can ascertain the level of obstruction. Sometimes a naso-pharyngeal exam is done with a flexible fiber-optic camera. To confirm the amount of cardiovascular compromise and decreased oxygenation levels, a sleep study may be recommended to monitor an individual overnight.

There are several treatment options available. An initial treatment may consist of using a nasal CPAP machine that delivers pressurized oxygen through a nasal mask to limit obstruction at night. One of the surgical options is an uvulo-palato-pharyngo-plasty (UPPP), which is performed in the back of the soft palate and throat. A similar procedure is sometimes done with the assistance of a laser and is called a laser assisted uvulo-palato-plasty (LAUPP). In other cases, a radio-frequency probe is utilized to tighten the soft palate. These procedures usually performed under light IV sedation in the office.

In more complex cases, the bones of the upper and lower jaw may be repositioned to increase the size of the airway (orthognathic surgery). This procedure is done in the hospital under general anesthesia and requires a one to two day overnight stay in the hospital.

I had experienced 7 weeks with another practice after going for an exam, x-rays, and cleaning. The experience finally included an extraction and placement of a bridge. The aftermath left an uncomfortable mouth, ill-fitting bridge, poor customer service and communication and a large measure of stress. At Dr. Genetti's office, the entire staff was extremely caring, accommodating, knowledgeable, competent, and professional. Even though she was cleaning up someone else's mess, I was her first priority and she strived for satisfaction. She even called me in the evening to see if I had any questions and if I was OK. The experiences between the two offices was like night and day. My mouth is now fine and I cannot say enough about the service and experience I had. It is so good to know there are competent and caring doctors in the profession. I would recommend this group totally and appreciatively.

This is the nicest dental office I have ever been to. I’ve always dreaded going to the dentist, but when I walked in the door, I knew right away this was the right place for me. The office itself is beautiful and feels and looks like a spa! Everyone I interacted with was extremely friendly and knowledgeable and they were all able to answer my questions. Dr. Genetti immediately put me at ease. Right away she asked me about my story and concerns and she really took the time to listen to what I had to say, which is so rare! At each appointment, I never had to guess what was happening because everything was explained so clearly. Dr. Genetti is an extremely consciousness and kind dentist and I could tell from the first exam what a confident and knowledgeable practitioner she is. The team works together so well and you can tell how much they care for their patients and each other. I was relaxed throughout my whole treatment because I felt I was in such good hands. I was recommended to you by a friend and pulmonary NP for sleep apnea appliance. During the examination the uneven lower front teeth were talked about. Dr Weierbach explained cosmetic countouring to me. I decided to have it done! I was amazed that a simple procedure could make such a visable difference to my smile. So sad that for a long time when out in public a mask had to be worn and smiles not seen!! I am very happy for sleep apnea appliance and even front lower teeth! Thank you
